Hameed is our clinical services manager, experienced in acute & complex health, mental health, community and the aged care sectors. Hameed graduated from Murdoch University with a Master’s in Healthcare Administration, Policy and Leadership and a Masters in Nursing Practice and Clinical Nursing from Curtin and Notre Dame Universities. He has over five years of experience in the NDIS industry with support work, service coordination and clinical services. Hameed has also been involved in academic research, clinical education, training and clinical services management. Hameed is in the process of completing his postgraduate degree in counselling and psychotherapy. As the Clinical Services Manager at Rise Community Care, Hameed brings extensive clinical knowledge and experience.
Meet our Support and service coordinator manager Jincy. As a clinical nurse who has worked in the hospital and aged care industries for more than 24 years, Jincy has a broad variety of knowledge and expertise. She has a strong commitment to doing good deeds in the community and enjoys guiding clients from all backgrounds via the NDIS support services. In her capacity as a referral coordinator and support coordinator, she gathers data on the client’s history and provides person-centred services through Rise Community Care.
Michael is a Senior Registered Nurse and clinical nurse with over 13 years’ experience, working in various Acute Medical settings at both Royal Perth and Fiona Stanley Hospitals. Michael worked in multiple Wards, including Acute Medical with High Acuity Care, Emergency, Short stay Medical Unit, Post Trauma Rehabilitation Orthopedics, Elective Orthopedics Surgical Unit, spinal ward and Older Adult Acute Care and community care. Michael provided patient centric health care during this time, he performed a broad range of different roles, including; managing advanced clinical skills and knowledge in delivering patient care, Chronic disease management, Chronic wound pressure sore assessment and dressing, nasogastric feeding, PEG feeding Indwelling and super pubic catheter insertion and management, bowel management, caring patient with diabetes and patients with noninvasive ventilation. Michael has a comprehensive knowledge of medications and can safely administer medications to patients in accordance with working knowledge of legislation and pharmacology.
Blessy currently working as a Geriatric clinical nurse consultant. Blessy has 25 years’ experience as a Registered Nurse both overseas and Australia. With years of experience in patient care, she has determination, compassion, and critical thinking ability that she can apply to all her responsibilities. Blessy has abundant experience in formulating implementing and evaluating Care Plans and Risk assessments for the residents as well as assess health status and manage care for participants with acute and chronic concerns. Blessy’s present experience in Acute Geri patients to provide high standard of nursing care, challenging clinical needs, mentoring junior nurses and has an achievement in Dementia Champion. Blessy’s career achievements: – Performance above and beyond the call of duty (2005), Nurse of the year award outstanding performance (2011), as a clinical mentor (2012). Employee of the month (2014 – Regis Aged Care). Dementia Champion (2020) clinical leadership program (2022) Blessy’s strong initiative and exceptional organizational skills enable her to make a substantial contribution to Rise Community Care and provide an excellent opportunity to best utilize clinical skills while contributing to community, participants and their families.
Fara has a wide range of knowledge and experience working in the disability and hospital sector for 10 years. She is passionate and enjoys assisting clients from multi diverse background in the wider community.
As a Registered Nurse, she gets to interact with her clients and gain a deep understanding of their needs and goals. Fara takes pride in her dual role Registered Nurse/HR manager and is committed to see Rise Community Care grow. Apart from this, Fara is also keen on helping in other areas such as IT operations, administration and strategic planning.
